How do I remove Level Quality Watcher?

It doesn't show on either the list from "Should I Remove It?" or "Change or Remove Programs". My firewall asked if I wanted it blocked is the only way I new it existed. So now, how do I remove it? This is what it said "C:\Program Files\Level Quality Watcher\v1.01\levelqualitywatcher32.exe"

That doesn't work for me because mine doesn't even give the option to remove it. I've been through five removal programs and nothing has taken it out. Time to format and do a clean install.
